Immunizations
The Immunizations Clinic is open Monday-Friday, 8 a.m.- 5 p.m. by appointment only in Texas City, 9850-B Emmett F. Lowry Expressway, Suite B-104. No walk-ins will be accepted. Please call 409.949.3459 to schedule an appointment. 

Customers will be asked to wear a face covering during their visit to the office. One will be provided if needed. Please do not bring family or friends with you. Only the patient will be seen.


Healthy Concepts Clinic
Healthy Concepts Clinic is open Wednesdays, 8 a.m.-5 p.m., by appointment only in Texas City, 9850-B Emmett F. Lowry Expressway, Suite B-107. Please call 409.938.2493 to make an appointment. No walk-ins will be accepted.

The Galveston clinic is closed until further notice.

TB

The TB Clinic is open Wednesdays, 8 a.m.-5 p.m., by appointment only in Texas City, 9850-B Emmett F. Lowry Expressway, Suite B-105. Please call 409.938.2217 to make an appointment. No walk-ins will be accepted.

Community Health Services improves the health and well-being of the community by encouraging, supporting and empowering residents to be healthy and self-sufficient.

We provide health and community education by request to schools, churches, civic organizations and health fairs. Request us at your event by filling out this form.

Community partners include: youth groups, homeless coalitions, senior service centers, county head starts, disaster and assistance agencies, health education centers, health promotion agencies, hospitals, private physicians, school districts, faith based agencies, civic organizations, churches, state and local health services, home health agencies and mental health services.
