To assist us in expediting your requests, the following procedure has been implemented. If you have any questions or comments, please call 409. 938.2251.

• Environmental Information Form
  All consultants/companies/citizens requesting information will be provided a “Environmental Information Form”, this form must be completed in its entirety. A map pinpointing the location of the property must be provided. References such as roads, streets, drainage ditches, creeks and bayous shall be included. Hand drawn maps are acceptable. Map shall NOT be a key map unless it is enlarged to at least 1 mile equals 4 inches
