Under the supervision of the Medical Director (for medical issues) and the 4C’s Business Director (for business/organizational policy issues), this position will provide healthcare services as a credentialed, licensed Physician Assistant or Nurse Practitioner to patients (on an appointment and walk-in basis) of the 4Cs Clinics in accordance with state and local laws, established clinic scope of services, and in accordance with priorities and goals adopted by the 4Cs Governing Board.

 

Essential Job Functions

95% -Interviews patients to obtain history, performs physical examination, orders lab and other tests, prescribes medications and treatments; provides continuity in managed care for patients with pre-existing long term problems; makes referrals for secondary and tertiary care; performs medical procedures, according to privileges issued; performs microscopy procedures and waived lab tests per protocol; provides health maintenance visits, evaluates for immunizations, and provides anticipatory guidance and referrals; performs contract and special physical examinations as Texas Health Steps / well-child visits, HeadStart,  employment, etc; performs STD (sexually transmitted disease) and family planning screening and education; documents all patient contact accurately and legibly on the medical record; performs as an active member of the medical team; works with Medical Director and nursing personnel to develop/implement/update protocols and procedures (including development of education materials, clinical protocols, such as telephone and/or on-site triage of patients).


5% - Other duties as assigned; including, but not limited to, participating on clinical committees; performing assigned home visits with or without a health care team; and providing 4Cs educational, quality review or other activities.

 

Knowledge, Skills, and Abilities

Knowledge of adult learning principles, ambulatory nursing practices and procedures, and community resources.

 

Skill in providing excellent customer service and support; organizing and prioritizing workload and meeting deadlines; excellent written and verbal communication; and nursing skills such as administering medications and other office treatments.

Education / Experience

Bachelor's degree from an accredited college or university.

Graduation from an approved PA or NP program.

Recognized as an Advanced Practice Nurse by BNE (N.P.)


Preference will be given to work experience in a public health or community clinic setting.


License and/or Certification

Current certification and licensure by the State Board of Medical Examiners or Nurse Examiners.

Current CPR

Valid Texas Drivers License


Special Requirements

Must have reliable transportation and be willing to travel between GCHD facilities.   Must be willing to work evening hours if necessary.
