Public Health Educator/ Patient Referral Specialist

Under the direction of the Program Manager of the Women, Infants, and Children (WIC) Program of Galveston County the Public Health Educator / Patient Referral is responsible for providing specialized health information, referrals, knowledge and skills to various community groups, the general public, and employees.

 

Essential Job Functions                                                                      

95% Plans, organizes, and develops health education programs that meet community needs.  Oversee WIC’ s obesity program, and provide lessons in the WIC sites; work with clinical managers to organize and schedule educational sessions in the WIC sites; provide health education in the schools; attend local community health fairs; organize and facilitate WIC’s annual National Nutrition Month and World  Breastfeeding Day celebrations and write articles to WIC news; collects data and performs research to develop, pretest and evaluate health related education techniques and/or materials.  Schedules and presents health education programs/seminars for community.  Establishes and maintains a close working relationship with the community, and makes patient referrals to 4C’s dental and medical clinics as well as other community resources.

 

5% - Other duties as assigned by Program Manager (i.e. participate in worksite wellness activities as requested)

Knowledge of basic public health information and routine business functions such as organizing, scheduling, maintaining lesson plans.

Education / Experience

Requires a Bachelors degree in Nutrition or Dietetics, Education, Behavioral Science or a closely related field such as Public Health.

 

Experience in training and developing education materials preferred.  Directly related professional experience in nutrition, dietetics, public health, education, or a closely related field, may be substituted for the educational requirement on a year-for-year basis.

 

License and/or Certification
Valid Texas Drivers License

 

Special Requirements

Bilingual (Spanish) required

Must have reliable transportation and be willing to travel between GCHD facilities.
