The “Take Action” A Diabetes Self-Management Program is a series of classes designed to help people with diabetes to understand how diabetes affects their body and how to make the best decisions regarding their management of diabetes on a daily basis. The emphasis is on learning to set goals to make life easier. Each class can be opened separately, and is in .pdf format, unless specified otherwise.
