The Diabetes
Self Management Program addresses commonly identified needs of those
with diabetes enabling them to properly manage their condition.
Through
collaboration with community agencies and individuals, the “Take
Action Galveston” curriculum was developed. It is a series of classes
designed to help people with diabetes understand how it affects their
body and how to make the best decisions regarding diabetes management
on a daily basis. Volunteers and health professionals are recruited
to become Diabetes Health Coaches to present community classes.

The program is
open to anyone who would like more information about diabetes.
Locations:
Education classes
and support groups are held in Galveston and Texas City. Call for class
schedules on the Bolivar Peninsula.
Cost:
Classes are $5.00 per session when held at Galveston and Texas City. There is no
charge for classes taught at other sites.
