The Diabetes Self Management Program addresses commonly identified needs of those with diabetes enabling them to properly manage their condition.

Through collaboration with community agencies and individuals, the “Take Action Galveston” curriculum was developed.  It is a series of classes designed to help people with diabetes understand how it affects their body and how to make the best decisions regarding diabetes management on a daily basis.  Volunteers and health professionals are recruited to become Diabetes Health Coaches to present community classes.
