The Community Health
Nurses (CHN's) are Registered Nurses that work in conjunction with numerous
Health District programs, such as case
management, and many community partners. They provide home follow up care,
immunizations for homebound patients, health education and referral
of clients
to appropriate agencies for assistance. The following
services are offered by the Community Health Nurses:
- Preventive health and community education is provided on request to
schools, churches, civic organizations and health fairs.

- Home Visits/Referrals: Follow up
care for missed appointments, chronic illness
education, nursing or home assessment, flu/pneumonia shots
for homebound patients
- Assists in the
formation of faith-based health initiatives, local coalitions,
coordinates and participates in
community groups.
Any Galveston County resident is
eligible to use the Community Health Nurses services

Hours: Community Health Nurses are available Monday-Friday, 8:00 am-5:00 pm.
Costs: Services are available free of charge.
Community partners Include:
youth groups, homeless coalitions, senior service centers, county
head starts, disaster and assistance agencies, health education centers, health
promotion agencies, hospitals, private physicians, school districts, faith based
agencies, civic organizations, churches, state and local health services, home health
agencies and mental health services.
