   New Animal Services Manager
Red diamond After the resignation of Michelle Reynolds, the Health District is pleased to announce the selection of Kim Schoolcraft as the New Animal Services Manager.  Kim is a former police officer for the City of Galveston and served as Chief of Police from 1997-2000. Later, she served as manager of the City of Galveston's Animal Shelter.  Kim loves pets and has volunteered at local shelters including the Health District’s Joe Vickery Animal Shelter.  Kim has a strong desire to reduce the number of unwanted pets in Galveston County through adoptions, public education, spay & neuter programs, and compliance with local and state regulations.  She assumed her new position on Monday, April 24th, and reports to Ronnie Schultz, Director of Environmental Health Programs.  Kim may be reached at 409.948.2485.  For more information, please contact Ronnie Schultz, Director of Environmental Health Programs, at (409) 938-2314 or email rschultz@gchd.org
 

  Meeting with Local Governments
Red diamond Dr. Mark Guidry and Ronnie Schultz presented the latest data from the ongoing investigation in the Lake Madeline area at a City of Galveston council workshop on April 13, 2006.  The City of Galveston decided to form a committee to explore solutions as well as decided to post warnings in the area.   District anticipates participating on the committee and continuing its efforts to assess and identify pollution sources. On April 26, 2006, Dr. Guidry presented the Strategic Health Plan’s 2005 Progress Report to a Galveston County Commissioners Court workshop.  The Judge thanked Dr. Guidry for the presentation and also for the leadership of moving forward with the Health Plan.  The Judge also sent congratulations to all staff that assisted in implementing the Plan.  For more information, please contact Harlan “Mark” Guidry, MD, MPH, Chief Executive Officer, at (409) 938-2401 or email mguidry@gchd.org
   
Joint Commission on Accreditation of Healthcare Organizations (JCAHO) Unannounced Survey
Red diamond On Monday, April 17th, the surveyors appeared, unannounced, to assess us and began their two day process.  Staff made significant and necessary adjustments in their schedules to accommodate the surveyors.  At the exit interview we were informed of continued accreditation!  The surveyors mentioned that the 4C's clinics remain in the minority of healthcare organizations and community health clinics who are JCAHO accredited.  Because of our commitment to healthcare quality and safety, we will continue to welcome and actively participate in the JCAHO process.

  City of Galveston Emergency Management Planning

  In preparation for hurricane season, EMS staff members have been meeting with the City of Galveston to address the evacuation of special needs persons in the event of a mandatory evacuation.  There are an estimated 100+ persons in Galveston who have special needs.  The City of Galveston is undertaking outreach efforts to identify these persons in advance of any declared evacuation.  There have been nearly 75 persons identified as of April 20, 2006.  Those persons identified, have provided information to surveyors and the 8 to 10 that may have a mobility related special need are being referred to EMS staff for evaluation of their need.  EMS is also involved in coordinating and cooperating in the evacuation of stretcher patients from local long term care facilities and hospitals as needed in a mandatory evacuation situation.  For more information, please contact Mike Carr, JD, Director of Emergency Medical Services at (409) 765-2524 or mcarr@gchd.org. 

 Pandemic Flu Readiness

  On April 6th, Dr. Mark Guidry and Planner, Brian Rutherford, met with Galveston Independent School District officials to discuss collaboration in two mass dispensing exercises. The exercises would involve using a school facility to simulate a scenario where antivirals are dispensed to the community. The first exercise, taking place in the late summer, would involve setting up a mass clinic site in the facility’s gymnasium. The second exercise would consist of setting up the clinic and having staff and trained volunteers man the clinic as volunteers from the community walk through as patients. Beach Party Weekend (April 22-23, 2006)

  During Beach Party Weekend 2006, Environmental staff issued 40 temporary permits to food service vendors, STD/HIV staff provided information about testing and distributed prevention packets; and GAAA EMS was ready on the scene and responded to 6 event related calls during the weekend.  For more information, please contact Pisa Lewis, Executive Assistant, at (409)938-2401 or email plewis@gchd.org.  

Probable Mumps

  During the month of April, Epidemiology Services received 2 reports of probable mumps in a 5 year old child and a 1 ½ year old toddler.  Both cases were unrelated and had symptoms compatible with mumps (fever and swelling of the parotid gland).  Serological testing for mumps was performed on the 1 ½ year old and yielded a negative result. Toddler was current on immunization.  No serological testing was performed on the 5 year old and child was not current on immunizations.  Mumps is a vaccine preventable disease that is transmitted through airborne, droplet spread or direct contact with the saliva of an infected person.  Last documented cases of mumps in Galveston County was in 1993 – 3 cases.  For more information, please contact Dana Wiltz-Beckham, DVM, Chief Epidemiologist, at (409) 938-2322 or email dbeckham@gchd.org

Red diamond Dates for upcoming mammogram screenings were submitted to the Community Bulletin Board in the Galveston Daily News with eligibility guidelines.  These sites are submitted to the newspaper on a monthly basis. Additional information on the availability of mammograms can be found on the District’s web site at  http://www.gchd.orgchn/dfeet.htm.

Food service establishments that received Gold Ribbon awards this year were recognized in the Applause section of the Galveston Daily News on April 3rd.

The District’s Joe Vickery Animal Shelter recently received a donation from the Whiskerville Animal Sanctuary of a microchip scanner.  The scanner will be used to help reunite lost animals with their owners.  A photograph of Ronnie Schultz receiving the donation was issued to local media on April 13th and can be seen at http://www.gchd.org/press/2006/04Whiskrville.htm.

District Animal Control Officers seized 20 dogs, two cats, one goat, and one pig from a home in Santa Fe on April 11th.  The Galveston Daily News was on the scene and filed the following report that was published in the April 12th Police Beat http://www.gchd.org/press/2006/04womancited.htm.  An update to the seizure was published on April 18th.  Ronnie Schultz and Kurt Koopmann contributed to the story which can be found at http://www.gchd.org/press/2006/04womancitedupdt.htm. After the case was heard in municipal court the owner was allowed to keep four of the animals along with the goat and pig.  The other animals were being offered for adoption or redemption to former owners at the Joe Vickery Animal Shelter.  An additional update following the hearing was published in the April 21st edition of the News and can be accessed at http://www.gchd.org/press/2006/04womancitedupdt1.htm.

With a change in the seasons we are beginning to see some of the years first advisories posted under the Beach Watch program.  When advisories are posted staff notifies community leaders and posts them on the District’s web site at http://www.gchd.org/pollution/BeachAdvisory.htm.

Staff attended a City of Galveston workshop to address the ongoing assessment of water quality in Lake Madeline on April 13th.  Information was presented to council members and public in attendance.  The Galveston Daily News ran a story on the Lake which can be found at http://www.gchd.org/press/2006/04LakeMadelineDNews.htm.

National Infant Immunization Week will be observed April 22-29th. Galveston Daily News reporter Kelly Hawes interviewed Dr. Mark Guidry, Dr. Abdul-Aziz Alhasson, and Governing Board member Mr. Willie Tolliver for an upcoming series on the health care challenges faced by the uninsured and underinsured.  The series is scheduled to run during National Cover the Uninsured Week in May.  After the interviews the reporter and a photographer were escorted to the Texas City 4C’s clinic where two patients agreed to speak with them concerning challenges they personally faced and how the 4C’s clinic help them meet their healthcare needs.

Red diamond Community Outreach Activities:  Community Health Services staff participated in four health fairs this quarter, reaching approximately 150 people with immunizations, 41 blood sugar checks, and 145 with health information.  A presentation was made to over 100 nursing students at the UTMB School of Nursing about Community Health Nursing.  STD services provided educational presentations to 344 persons during the month April.

Senior Health:  At the April 11 Senior Health Advisory Council meeting, it was announced that the Galveston County Commissioner’s Court proclaimed May as Older American’s Month.  The council is currently working on three projects: the 50+ Expo at Mall of the Mainland on 5/4/06, Walk Across Texas, and GCHD Volunteer Recognition Week events.  Walk Across Texas will kick-off with the Senior Health Walk at Carbide Park on 5/31/09 and conclude with the Recognition Ceremony at the Doyle Convention Center on 8/8/06.

Robert Wood Johnson Building Community Supports for Diabetes:  The diabetes group developed a new pamphlet which describes the eight on-going diabetes education classes and the three on-going support groups.  On April 20- 21, the Diabetes Program staff presented a Coach Training class for both the Take Action (diabetes education) and Whisking Your Way to Health programs for 19 Area Health Education Center (AHEC) volunteers from across the state.  One of our RWJ project staff has been invited to serve as a member of a panel of three people to speak on the topic Models for the Use of Community Health Workers in Diabetes Self Management at the American Association of Diabetes Educators conference in August 2006.

Women, Infants, and Children (WIC):  WIC had and seized the opportunity to apply for childhood obesity prevention funding from DSHS. The proposal is to implement a series of childhood obesity prevention sessions to inform/educate the Galveston community and WIC participants about this issue.  As part of this project, WIC staff would provide obesity prevention educational sessions that focuses specifically on making healthy food choices and forming new coalitions with schools to assist in creating a school environment that promotes healthy nutrition and encourage physical activity habits.  There is an additional proposal to implement a series of yoga demonstrations and yoga health education sessions.  If funded, both programs would begin mid-May 2006 and end September 30, 2006.

STD/HIV Services:  The HIV/AIDS Surveillance Program was placed on Level I Sanctions after the site review in February 2006.  Findings mainly are related to incomplete data or data not received in a timely manner.  Progress has included changes in systems (having GCHD staff pick up lab work rather than waiting for it to be mailed, cross-training, using the TXHAN system), examining time management (workload analysis), and requesting clarification from the Texas Department of State Health Services (DSHS).  Further assistance may be needed from DSHS with regard to coordination between other reporting entities. Currently, the backlog is expected to be eliminated by April 26.

Red diamond Animal Seizure – Responding to a citizen complaint, County Animal Control Officers armed with a court approved seizure warrant removed 20 dogs and other animals and issued a Santa Fe woman 41 citations.  The citations issued were for being over the legal limit, not having proof of rabies vaccinations or county registration.  The case was heard on Wednesday, April 19th in the local Justice of the Peace court.  The presiding judge ruled in favor of the Health District and awarded possession of all but four of the dogs to the District.  The District was ordered to redeem the dogs to the rightful owners or adopt them to responsible families.  As of today, all of the animals with the exception of 4 dogs and 2 cats have been redeemed or adopted.

Gold Ribbon Awards - On April 5 our Gold Ribbon Awards for 2006 were presented at the Wayne Johnson Community Center in La Marque. This year, 62 establishments were recognized for their cooperation in maintaining high sanitation standards in these establishments. County Judge Jim Yarbrough and Harlan “Mark” Guidry, MD, MPH, CEO, presented the awards. The function was well attended and a good time was had by all.

Air Pollution Services State Audit – The Texas Commission on Environmental Quality (TCEQ) conducted an annual audit on April 4th of our Air Pollution Services Division.  The audit is mandated by the current TCEQ contract held by the division of which selected air services are provided.  Auditors reviewed the program’s filing system, record keeping system, enforcement policy and procedures, air investigation reports documenting compliance with air rules and regulations, and compliance with established TCEQ procedures.  The initial findings are all favorable.  A written report will follow.  The Air Pollution Services Division provides citizen complaint response, comprehensive compliance inspections, vapor recovery equipment inspections and permit review services under this contract.

Red diamond  4C’S Counselor Vacancy- The 4C’S Counselor position is now posted.  The Medical Director and Business Director are aggressively reviewing applicants; interviews are underway for this position.

Electronic Medical Records (EMR/CMS) - An evaluation team as been assembled to review & assess clinic needs.  The EMR Evaluation Team is drafting and preparing a functional packet to present for future contract bids.  Electronic Medical Records will aid continued efforts to provide the best possible service to Galveston County Residents.

Dental Clinic – The Dental Clinic is gearing up for a new contract dentist.  The dentist will provide 2 days of service to the dental clinic (one day at each site) once the credentialing process is completed.  

Medical Records Pilot Project:  Preliminary construction is in progress.  The projected target date is early May to implement project completion. The Medical Records Pilot Project will include redesigning the flow of medical records and centralizing the location of records and data entry clerks. 

Texas City Renovations – The next step with architectural contractors are to finalize drawings, evaluate contractors and construction timelines.  The Electronic Medical Records System will follow soon after the completion of the clinic renovation.    

TACHC Credentialing Web cast – Texas Association of Community Health Centers presented a web cast on credentialing & privileging medical providers. Credentialing Coordinators & Human Resources were in attendance for the broadcast.  The Texas Association of Community Health Centers (TACHC) is a private, non-profit membership association that represents safety-net health care providers in the state of Texas.

Grant Award – On March 26, 2006, the 4C’s Clinics received its Notice of Grant Award (NGA) for the period 4/01/06 to 3/31/07.  The award of $1,609,268, a reduction on 1% from FY05, is a result of a reduction in Congressional appropriations.  The good news is that the Clinics have been redeemed under the Federal Tort Claims Act and there are no conditions associated with the Notice of Grant Award. 

How Understanding Culture Can Reduce Health Disparities –  In an effort to improve communications with Healthcare Providers and 4C’S Patients, TMF Health Quality Institute provided our Medical and Administrative staff with training to better understand diverse cultures and how health disparities can be reduced.  One of the benefits of cultural competency training is to improve the quality of service as well as responding to current and projected demographic changes.  Cultural competency adds awareness, knowledge and skills to better treat our increasingly diverse patient population. To providers attending the training session, 1.2 contact hours were given for CME credit.

Red diamond ·GAAA Transfers - Total ambulance transfer calls were 347 in February and 401 in March.  Mainland transfers accounted for approximately 55% of the total calls, while Galveston transfers accounted for approximately 45% of the total. In February (79%) and March (76%) of the Mainland transfer calls were turned over to other services.  In Galveston, 2 transfers were given away from UTMB during this time.  Once all ambulances have been placed in service on the mainland, the number of transfers being turned over to outside agencies should decrease.  We will continue to monitor these numbers monthly.

EMS expansion update – Expanded EMS operations continue on the mainland.  In addition to current emergency operations, an additional transfer unit (Medic 12) has been placed in service on a 24 hour basis, as of April 1, 2006.  This unit is dedicated to mainland transfers and is operated from the vicinity of Mainland Medical Center during the day and housed in our Bacliff EMS facility at night.  An additional daytime transfer unit is planned for deployment as soon as staff can be acquired and trained.  Three new Type II transfer ambulances have been received in the last two weeks along with a Type I Ambulance that will be stationed in the Bacliff area as soon as it is commissioned and ready for service.  Two new Cardiac monitors and four Semi-automatic External Defibrillators have been received and made ready for use in the expansion.

Nursing Home Contact – EMS Director Michael Carr has continued to meet with representatives of local nursing homes to discuss issues involving EMS that directly affect these facilities.  He recently met with the interim management staff of Seabreeze Nursing and Rehabilitation Center to inform them of the changes to EMS operations that have occurred in the last year.  Another meeting will be arranged when the new permanent administrator reports to work in May.

West End Tele-health – On April 17, 2006 Michael Carr, EMS Director and Russell K. Miller, MD, EMS Medical Director, met with representatives of the City of Jamaica Beach, the Pirates Beach Homeowner’s Association, the West Island Resident’s Association, and UTMB, to discuss a proposed pilot program implementing a Tele-health Clinic to be located in the Jamaica Beach municipal facility.  As proposed the clinic would operate for four hours each Saturday and Sunday and Holiday from May 27, 2006 to September 4, 2006.  The participants agreed that they would seek funding for the pilot from their respective organizations.  These meetings are scheduled for early May, and if successful, the group will meet to determine the details of putting the pilot in operation on the Memorial Day weekend.

Red diamond On April 4th and 10th, the Public Health Planner attended meetings at the Houston-Galveston Area Council (H-GAC) to discuss possible recommendations for a regional evacuation plan. The recommendations specify the roles and responsibilities of various local and state agencies in the event an evacuation is needed. The recommendations come in response to the State of Texas Governor's Executive Order mandating regional planning for evacuations.

On April 6th, the Public Health Preparedness Program hosted a satellite broadcast on conducting time studies of mass antibiotic or antiviral dispensing clinics. The purpose of these studies is to evaluate through exercises a community’s ability to rapidly dispense needed medications to their residents. The goal is to use this information to improve performance and efficiency of a mass dispensing operation. Community Health Nursing and Public Health Preparedness staff viewed the broadcast.  

On Friday, April 7th, the Galveston County Animal Issues Committee consisting of Health District staff along with representatives from the Galveston County Office of Emergency Management, League City Animal Control, the Texas Agricultural Extension Office in Galveston, City of Galveston Animal Control, and Habitat for Horses, met to identify possible resources to address animal needs in the event of a hurricane. Committee members will be working to develop a resource list to assist livestock and pet owners and emergency responders to prepare for and respond to animal issues that might arise as a result of a disaster in Galveston County.  

On Saturday, April 8th, The Senior Health Coordinator, Public Health Planner, as well as staff from the Case Management Program participated in an event at the Island Community Center to educate and register seniors in the new Medicare Part D prescription benefit. 130 seniors were provided assistance. The event was organized by the Galveston Community Network; an organization consisting of the Health District, churches, social service organizations and local agencies that provide services to Galveston Island residents. Besides providing assistance on the day of the event, the Health District also provided support by printing flyers and signage for the event.

The Galveston County Emergency Response Collaborative (GCERC) met on Thursday, April 13th. The Public Health Planner and Strategic National Stockpile (SNS) Coordinator provided a presentation to orient members to the SNS and how medications would be received and dispensed to residents and hospitals in the county. There was also discussion about the command and control structure and the various roles and responsibilities of member agencies.

Also on April 13th there was a meeting of the Galveston County Medical Reserve Corps. Members were provided a brief overview about pandemic flu planning in the county and possible roles of the Medical Reserve Corps in a SNS deployment in Galveston County.  From April 18th to April 21st the Public Health Planner, who also functions as the Galveston County Medical Reserve Corps Coordinator, attended the National Medical Reserve Corps Conference in Dallas. The purpose of the conference was to share best practices on volunteer recruitment, retention, training, and community collaboration. Approximately 500 local and regional coordinators from throughout the country attended the conference.

Red diamond Influenza Update - There have been 42 laboratory confirmed Influenza cases reported to the Health District in Galveston County for the year 2006.  Overall, flu activity level for Texas is sporadic and continues to decrease across the state. 

Meetings - Epidemiology staff attended the Bugs, Drugs and Vaccine:  Securing our Future symposium on April 5 and 6 designed to attract young investigators to interact with their peers and Texas science community.  On April 5, 2006, staff attended an Environmental Health for Public Health Department Epidemiologists Training at The University of Texas School of Public Health in Houston and represented the Health District at a meeting about Texas Pandemic Influenza Supplemental Funds meeting to explore ways all areas of their region will become prepared to address a pandemic influenza.  The Health District anticipated additional funding specific to pandemic fund planning.

Volunteers - During the month of April, 34 EMS volunteers were processed, one medical student was assigned to WIC, and a phlebotomy extern was placed in the clinic lab. The volunteer coordinator attended the following meetings this month:  Volunteer Management presented by the Houston Area Volunteer Administrators, and on April 13 she participated in the Texas City Healthy City Assessment project and the monthly Galveston County Medical Reserve Corp meeting.  She also assisted ten seniors at McKinney Memorial Methodist Church with enrollment for the new Medicare Prescription Program.
