2005 Annual Progress

Executive staff
are gearing up to prepare a 2005 annual report of progress toward the Strategic
Health Plan's 8 District priorities. The annual report will include an
executive summary, a detailed report of accomplishments, and a PowerPoint
presentation. A few selected highlights of 2005 include: 4C's clinic Redesign;
preparations for a GAAA mainland expansion; improved preparedness for public
health emergencies; Response to Hurricane Katrina evacuees; Hurricane Rita
evacuation and response; improvements in public awareness and community health
advisories; many operational improvements; and much more. Executive staff
anticipates having the report ready in late January for presentation, review,

High Island Sewage Issues |
 |
Dr. Guidry, CEO, Jay Holland, COO, Ronnie Schultz, Director of Environmental
Programs, and Marty Entringer, Consumer Health Manager, briefed the
Commissioner’s Court on December 12, concerning a non-compliant collection and
treatment system in the High Island area. The Health District reported that 32
homes are tied into a makeshift sewer system that needs to be disconnected and
the current residents connected need to be placed onto a treatment system that
meets today’s standards. This partially treated waste raises health concerns and
possible health effects such as diarrhea, weight loss and more severe symptoms
such as vomiting and fever may occur. The District will seeking voluntarily
compliance of the 32 homeowners recently identified as connected to the faulty
system. In addition, the District has collaborated with the County’s Grant
Coordinator, the Texas Commission on Environmental Quality and the Houston
Galveston Area Council to seek longer-term, community-wide solutions to the
problem. Finally, the District will continue to work with county elected
officials to evaluate and resolve the issues. For more information, please

Varicella Cluster |

The Epidemiology Services is monitoring a varicella cluster that so far includes
13 Texas City/La Marque children from Intercity Elementary. These children
became ill between 11/28 and 12/8. The children range in age from 6 to 11
years old. All children had been previously vaccinated against varicella, with
a wide range of vaccination dates. Of the 6 children for whom vaccine lot
numbers were available, no two lot numbers were the same. After discussions
with DSHS Department of Vaccine Preventable Disease and the school nurse at
Intercity Elementary, Epidemiology Services has decided to watch for additional
cases, and see if the impending holiday break disrupts the cluster of cases. If
evidence of transmission continues, students at the elementary could be advised
to seek an additional dose of varicella vaccine. DSHS Vaccine Preventable
Diseases noted that the varicella vaccine is very sensitive to temperature
changes, and care should be taken to ensure that it is always stored properly.

Influenza Update |

As of December 19th, the Immunization Department
has administered 4051 flu shots, an additional 700 shots were administered by
the 4C’s Clinics. 1851 of these shots were given to seniors. Epidemiology
services reports no confirmed or reported cases of influenza in the County for

Public Information staff diligently worked to produce the District Employees’
Family & Friends Cookbook for distribution before the holidays. The
project completed in-house contains recipes submitted by employees and is
available for purchase at five dollars a copy. Funds raised will be used for
employee activities. Dates for upcoming mammogram screenings were
submitted to the Community Bulletin Board in the Galveston Daily News
with eligibility guidelines. Information on the availability of mammograms can
be found on the District’s web site at

A call was received from Kelly Hawes with the Galveston Daily News on
December 14th working on a story concerning on-going sewage issues
in the High Island community where 32 homes have been determined to have
non-compliant sewage disposal systems. Public Information officer Kurt Koopmann
and Environmental Programs Director, Ronnie Shultz provided information in the
interview. On December 15th a second interview was conducted to
answer additional questions posed by the News reporter. The story

UTMB OAT Fall Workshop -- As part of the 4C’s ongoing participation in
the UTMB OAT Collaborative, staff attended a workshop held at Open Gates in
Galveston. The OAT Collaborative is identifying and eliminating health
disparities by the use of electronic means. A Health Resource Services
Administration (HRSA) grant funds this collaborative. The group heard reports
from the three groups participating, UTHSC, Tyler; UTHSC, Brownsville, and UTMB,
Galveston. Each group outlined their process for addressing the health
disparities in their communities through technology assisted methods. The
Galveston group, of which the 4C’s is a member, is pursuing a method to share
patient care data among providers who see the same patients via electronic
means. Participants include, the 4C’s, UTMB, St. Vincent’s House, The Jesse
Tree, and Mainland Medical Center. Funding for continuing the program through
the second and third years is currently in question and will be based on
Congressional authorizations expected in the next month. |

Advanced Animal Control Training – Five of the District’s Animal Control
Officers received advanced Animal Control Officer Training and completed the
certification test. Advanced officer training is offered by the Texas
Department of State Health Services and is designed to assist animal control
professionals to advance their knowledge, animal control techniques, ethics and
attitudes. Students have to prepare and present two research topics during the
training class. Certification tests include two written exams and a practical
test that requires students to identify breeds and give the two presentations.
This training helps to fulfill our commitment to proper training of our animal
control staff as identified in a past internal audit of animal services.
Holiday Giving – The staff of the Office of Environmental Health Programs
adopted two (2) families at the Women’s Crisis Center for the Holidays. With
the monies collected, we were able to purchase multiple gifts for the children
and their moms to help comfort them during this holiday season. A special
thanks to Renee Boles, administrative assistant, Air and Water Pollution Control
Division for coordinating this effort and making sure that the presents were
bought, wrapped and delivered. |

Senior Health Advisory Council -
The Senior Health Advisory Council met on December 13, 2005. During the
meeting, the Council reviewed the goals that were set for 2005 and noted that
they successfully completed them all. For 2006, the council chose to focus on
improving, refining, and enhancing projects that have already been started.
Projects suggested for 2006 included a Senior Summit/Expo, cooking schools in
the senior centers, an organized and sustained physical activity program, the
spread of a Train-the-Trainer model of informing seniors about Medicare Part D,
and working with emergency management personnel regarding the evacuation needs
of seniors.
HIV Services Audit - Only one portion of the “Administrative Agency”
site visit was performed on December 14. Computer data entry protocols and
practice were reviewed. All other components of the Administrative Agency and
Case Management are expected to be reviewed in January 2006. One plan and a few
policies will need to be written to be in compliance. Additionally, some of the
computer certificates need to be re-issued. The rest of the site visit time was
spent providing technical assistance to our new HIV Services Program Manager,
Mitchell Rappaport. |

Mainland Expansion Contracts – The Health District has
received and is reviewing four draft contracts necessary for the mainland
expansion. Once the contracts are finalized, they will be brought to the Board
for approval. 2005 Service Calls Expected to Break Records – GAAA is
poised to shatter records for emergency calls and total call volume. November
was a record month with 834 EMS calls on the Island and an additional 295 EMS
calls on the Mainland. This gives EMS a total of 9,343 calls on the Island and
12,826 total calls, already surpassing the total call record of 12,760 in 1997.
The year 2003 had the previous record with 10,096 EMS calls on the Island and
GAAA is less than 200 calls from that total on December 20, 2005. |

Star Performers - Twenty-eight employees received “Star Performer" awards
at this years' Kemah Holiday Jamm on December 10, 2005. Executive
Managers gave awards to non-management employees who demonstrated exceptional
performance over the past year. Volunteers Program - The volunteer
coordinator attended the quarterly diabetic coaching meeting on December 7th
at the Island Community Clinic. During this activity, three volunteers were
recruited to provide diabetic education classes in the community. A summary of
volunteer recruitment, placement and the dollar value of volunteer hours
contributed, was presented to the Senior Health Advisory Council on December 13th.
To date there have been 234 volunteers recruited which represents over $174,886
worth of volunteer time to the Health District. A Train the Trainer workshop on
the new Medicare Prescription Program was presented to six nurses at Greater
Bell Zion Baptist Church on December 13th at 7 p.m. They will be
available to assist the members with the enrollment of the program. The
volunteer coordinator taught the Sit and Be Fit exercise class to 11 seniors at
McKinney Memorial Methodist Church on December 6th from 10:00 a.m. to
10:30 a.m. During the month of December 15 volunteers were recruited and
placed in various departments. To date there have been 234 volunteers recruited
for 2005.
Public Health Preparedness Meetings: (1) technical Assistance Conference
on December 6th and 7th, to discuss the requirements and
deadlines of the Public Health Preparedness Grant; (2) a regional meeting
of the Medical Reserve Corps (MRC) on December 8th and 9th
to share best practices and discussed strategies to increase membership and
training opportunities; (3) a conference on “Protecting Our Community
from Influenza" on November 29 which highlights public
health strategies to prepared for avian flu or an
influenza pandemic; (4) a regional Strategic National Stockpile (SNS) meeting on
December 13th, to discuss sharing resources to enhance readiness for
mass vaccination or mass antibiotic dispensing clinics; and (5) a meeting with
Centers for Disease Control and Prevention, US Customs and Border Protection,
Cruise ship officials and local health departments (Houston/Galveston and Harris
counties) on December 14th , to discussed protocols regarding issues
of communicable diseases, quarantine, and cruise ships. |
