  Influenza Vaccination Efforts

red diamond pic The Immunizations Department began administering flu vaccine at the Senior Health Summit on 10/26/05.  The Texas City and Dickinson Immunization clinics were re-located to La Marque from November 1 to the Thanksgiving holidays.  A total of 3,595 flu shots have been given.  Flu vaccine clinics have been held in these community locations: senior citizen centers, long-term care facilities, churches, the Galveston County courthouse, and in a home for homebound citizens. In addition, the 4C’s clinics administered over 400 shots.  Epidemiologists report that flu activity throughout the state of Texas is sporadic.  The main influenza strain circulating in Texas has been typed as Influenza A/H3N2.  Galveston County has no reported and confirmed cases of influenza.  The Health District has received 126 counts of Influenza Like Illness (ILI) in the county.  ILI (fever over 1000F and cough and/or sore throat) activity in the county is assessed by sentinel providers, ISD school passive surveillance, and daily monitoring of over the counter drugs purchased at participating pharmacies throughout the county.    Avian Influenza/Influenza Pandemic Presentation
red diamond pic On November 21st, Chief Executive Officer, Dr. Mark Guidry; Chief Epidemiologist, Dr. Dana Beckham; and Public Health Planner, Brian Rutherford, conducted a workshop for Galveston County Commissioner's Court to discuss pandemic flu planning in the county. The three provided information about avian flu, outlined the potential threat to Galveston County residents, discussed current planning efforts, and presented information about disease surveillance capabilities between the Health District and its public health partners. The County Judge and Commissioners were each provided a draft copy of the Pandemic Flu Plan for Galveston County, general information about avian influenza, and information about the Galveston County Medical Reserve Corps.    Recruiting Members for Medical Reserve Corps

As part of District efforts to be prepared for pandemic influenza and other outbreaks requiring mass vaccination, the Health District is recruiting professional and lay volunteers to assist such an endeavor estimated to require some 2300 volunteers.   Dr. Guidry sent letters to elected leaders, hospital administrators, emergency managers and other community leaders, to introduce them to the Galveston County Medical Reserve Corps and request their support in identifying volunteers.  By signing up as member, one will be notified of the need for volunteers and of related training opportunities.  A brochure accompanied the letter, which highlighted the purpose of the Medical Reserve Corps and provides examples of how individuals can assist during a public health emergency.  Information about the Corps can be found at http://wwwgchd.org./bt/btindex.htm#medcorp  For more information, please contact Brian Rutherford, Public Health Planner at (409) 938-2275 or email brutherford@gchd.org

 District Presentations to Community Organizations

red diamond pic  Public Information staff worked with other programs in promoting flu shots in Galveston County.  The District’s web page www.gchd.org carried a promotional item with links to flu information.  On October 26, 2005 a press release was issued encouraging residents to get a flu shot. The release also contained helpful tips on how to prevent the spread of flu.  The release can be found at www.gchd.org/press/2005/10Flushot.htm. Stories promoting the flu shots were printed in the Galveston Daily News and also Gulf Coast E-News.

The District’s web master closely worked with Public Health Preparedness staff to promote the newly formed Medical Response Corps.  A letter encouraging local residents to participate was posted on the web site at http://www.gchd.org/bt/letter of Support MRC Volunteer 1105.pdf.  A promotional brochure was also designed that can be used to promote the Corps in the community.  The brochure can be found and printed out at www.gchd.org/bt/1105mrcbrochure.pdf

After an incident in the Houston area involving fake flu vaccine being given to area residents, some questions arose concerning the integrity of vaccine available in Galveston County.  On November 1st the Galveston Daily News ran a story on the subject and the District’s Public Information Officer was quoted assuring the public that all vaccine supplies are safe.  The story can be found at www.gchd.org/press/2005/11FlushotsRealDNews.htm.

The 4C’s Clinic’s Open House was the subject of a press release issued November 7, 2005 and posted on-line at www.gchd.org/press/2005/11OpenHouse.htm. The open house was held on November 16th in the Galveston 4C’s Clinic and was a success with Governing Board and United Board of Health members participating along with citizens from the local community.

red diamond pic 4C’s Redesign – The 4C’s redesign process has reached another milestone.  The model has been refined to the point that it has been consolidated into a handbook.  At the most recent in-service training session, clinic staff was introduced to the handbook.  The clinic now operates under the guidelines included in the handbook.

4C’s Open House – On November 16, 2005, the 4C’s hosted an open house in the Galveston facility to celebrate the finalizing of the redesign process.  Members of the 4C’s Governing Board and the United Board of Health spoke to clinic patients and 4C’s staff about the positive impressions they have been receiving about the redesigned clinic operation.  Several patients took the opportunity to receive their flu shots during the open house.  In addition, staff members were available to describe clinic operations to patients and others who toured the clinic facility in Galveston. Immediately following the open house, staff enjoyed a holiday pot-luck luncheon.

Partnership with Gulf Coast MHMR - On November 14, GCHD staff attended a meeting with the Gulf Coast Center (MHMR) and their partners regarding a  $1.6 million HRSA grant to improve mental health access in Galveston and Brazoria Counties.  The Gulf Coast Center plans to hire four case managers who will be placed at the Brazoria County Jail, the Luke Society Saturday Breakfasts, St. Vincent's House, Jesse Tree, UTMB Emergency Department and the two 4 C's Clinic sites.  These case managers will be mental health professionals who already have experience in this field.  They will work to coordinate the care of patients with mental health issues through a shared database that will be developed as part of the grant.  Patients with known severe mental health problems will be referred to the Gulf Coast Center, those with mild to moderate issues will be referred to 4 C's for primary care including some mental health treatments.  Grant staff plan to have these case managers hired by December.

red diamond pic Animal Advisory Committee Meeting – The Galveston County Animal Services Advisory Committee was formed to assist the Animal Services Division by reviewing and recommending necessary changes in operations, budget matters and compliance with all applicable laws, rules and regulations.  The first meeting was held November 1, 2005 with excellent participation from each entity and interest group represented.  During the meeting, the committee identified possible solutions to assist Animal Control Officers, peace officers, as well as the general public. These solutions include: development of a one-page guideline for police dispatchers on how and when to contact animal services after-hours, setting up training of police officers on the proper methods of restraining an animal, providing information on the animal services budget to include revenue sources, providing cost of purchasing an incinerator, researching the possibility of filing animal control violations in municipal courts instead of the current method of filing in the Justice of the Peace court, and the formation of a sub-committee to draft a policy on pet adoption and sterilization.  In addition, the committee voted on and approved the meeting schedule for the remainder of fiscal year 2006.

Air and Water Pollution Services – The Air and Water Pollution Services Division is currently drafting its work plan for the coming year to address a multitude of environmental issues.  This work plan is drafted in response to the many local, state and federal contracts that the division operates under.  These contracts require the Division draft a work plan to respond to citizen complaints, inspect facilities for compliance with environmental rules and regulations, and conduct air and water pollution sampling. In addition, the Division is drafting a work plan beyond the contractual requirements that include drive around investigations at pollution sources with actual emissions to proactively identify and resolve these air issues, conduct “backtracking” investigations to identify and resolve the source of water pollution as identified during routine sampling, and to review air monitoring needs in Galveston County and recommend sampling methods and potential funding sources.

Animal Control Officers course – On November 16, 2005, the Health District hosted a two-day basic animal control officer course for 21 participants representing Galveston, Brazoria, Harris, Matagorda and Dallas counties.   The basic animal control officer’s course reviews the Texas Administrative Codes as it relates to animal shelters, rabies, and animal control.  For more information, contact Dana Wiltz-Beckham, DVM, Chief Epidemiologist, at (409) 938-2322 or email dbeckham@gchd.org

red diamond pic Senior Health Advisory Council - The Senior Health Advisory Council met on November 8, 2005.  Two new members were added to the council: Harold Fattig and Eva Neinas.   Senior community assessment data as well as progress toward meeting 2004 goals and objectives were reviewed at this meeting.  At the December meeting, the plan of activities for 2006 will be developed.

Women, Infants and Children (WIC) – The WIC program was audited by the Department of State Health Services on October 26-28 as an accelerated monitoring review.    At the exit interview the auditors praised the program for significant gains in several areas including the number of clients with midpoint screening and participation by clients in classroom education.  The auditors also recommended looking at some of the WIC services closely in light of budget constraints, for example school-based services (reach few clients) and the need for more hours from a registered dietician.  Only three findings were described during the exit interview. The final written report is expected in the next 45 days.  There were no Rider 11 Child Abuse Reporting findings at this audit. 

State Audit of HIV/STD Program - The State audited the HIV prevention program November 8 – 11. The State auditors looked specifically at the Turning Point classes, held in Angleton and La Marque, and the Real AIDS Prevention Project (RAPP), conducted in the Cedar Terrace Housing Development in Galveston. During the Exit Interview, the auditor’s commended both programs and staff’s ability to “connect” with their subjects. The auditor’s only recommendations were to reduce the number of participants in the Turning Point Classes to 15 participants or less to maximize class effectiveness, have two sets of informational videos, and to use PowerPoint presentations and a projector instead of easel and posters. Although the final written report has not been received, there were no findings.

red diamond pic Mainland Area still finalizing contracts – GAAA is waiting on the new stakeholders to sign the appropriate contracts to provide service. The EMS advisory committee is meeting to develop the cooperative system. GAAA is continuing to plan for the additional stations, staff, equipment, and vehicles that will be necessary.

Texas EMS Conference – GAAA attended the State EMS conference November 20th 23rd, one of the largest EMS conferences in the Country. GAAA set up a booth to explain the new cooperative EMS system and to recruit new staff. Close to 120 applications were distributed. Staff also attended more than 100 hours of continuing education.

Lone Star Motorcycle Rally – GAAA’s ambulance and bike team provided added coverage for the Lone Star Bike Rally held in Galveston, November 3rd through the 6th. Responses were fewer this year than last even though attendance was higher.

Pre-hospital Care Committee – GAAA hosted the Pre-hospital Care Committee of the Gulf Coast Regional Trauma Council on November 3rd. The main topic was the Hurricane Rita evacuation. The meeting was attended by Galveston & Brazoria County EMS administrators. Brazoria County suffered many problems trying to evacuate ambulance patients. It was discussed that a regional plan needed to be developed and that the Galveston County operation, which went very well, should be used as a model for the region.

red diamond pic Annual Audit - Null-Lairson is currently performing our annual audit for the fiscal year ending September 30, 2005.  Interviews have been conducted with Board members and staff regarding transactions during this period.  The fieldwork is estimated to be completed in December, and a draft audit presented to the Finance Committees during the first part of February 2006. 

Title XX Funding -   The Health District recently noted that it was not awarded continued Title XX funding for eligible family planning and well-woman services in the 4C’s Clinics.  Pervious funding amounted to approximately $147,000.  The Compliance Officer is exploring the reasons why the Health District’s application was not awarded.   Should efforts confirm an anticipated loss of funding, executive staff will brief the board in a future meeting on a plan to balance the budget shortfall, to include any possible impact on services and staffing.

 Volunteers Program The volunteer coordinator gave a presentation on volunteer opportunities at the Health District and distributed the Health District brochures to 30 Alpha Kappa Alpha women on October 29th.  On November 2nd, Shirley Carr coordinated a workshop presented by Charles Kelly on The new Medicare Prescription Drug Coverage at Greater Bell Zion Baptist Church where 18 Medicare recipients were in attendance. Flu outreach clinics were provided for seniors at Greater Bell Zion Baptist Church and McKinney Memorial Methodist Church on November 7th and 8th.  Both outreach activities were open to the public. Three senior volunteers assisted the volunteer coordinator with this activity. The volunteer coordinator attended the 28th annual Governor’s Volunteer Leadership Conference, November 9-10, in Addison, Texas. The title of the conference was “Renewing Our Communities”. At the Governor’s Luncheon, Governor Rick Perry thanked volunteers for their support during hurricane Katrina and Rita.  Also during the month of November, five volunteers were recruited.

Galveston County Emergency Response Collaborative The Galveston County Emergency Response Collaborative (GCERC) met on November 10th. In this meeting, the FBI Weapons of Mass Destruction Coordinator for the Greater Houston Region met with members to discuss the role of the FBI and local law enforcement in the event of a terrorist attack. The group also discussed how a flu pandemic might affect Galveston County and the capacity of healthcare and public health to respond to such an event.

UTMB Presentation On November 14th the Public Health Planner conducted a presentation for UTMB Preventive Medicine Students. The students were given an overview of public health threats and preparedness activities in the county. As a result of this discussion, six of the medical students volunteered to assist in a public health emergency by joining the Galveston County Medical Reserve Corps.  

Mental Health Emergency Response - On November 15th, a meeting was held with mental health service providers in the county to discuss how we might coordinate mental health planning and response to a public health emergency. Representatives from UTMB, Red Cross, Gulf Coast Center, Devereux Treatment Center, Galveston County Emergency Management, and the Health District were represented at the meeting. Participants decided to conduct a conference in the spring to educate mental health providers on the role of mental health professionals in a disaster and to coordinate planning. Another meeting will be held on January 18th at 10am to discuss next steps in planning.
