Court-Ordered Treatment for Tuberculosis |

In his role as county
health authority, Dr. Guidry utilized laws of the state
and county for court-ordered treatment of a homeless man
on Galveston Island who was determined to be a threat to
the public’s health. State and county officials and the
man’s treating physician at UTMB concurred with the
determination made through an affidavit of medical
evaluation by the local health authority. County
attorneys initiated local legal steps leading to the
court order for long-term treatment in a state TB
hospital in San Antonio. Health District staff, with the
assistance of a county deputy, transported the man to
the hospital where he will be treated until he is cured
of TB. Most persons with TB are fully compliant with
necessary treatment and precautions to prevent spread to
others. The last time a court order was necessary was

| Exercising Public Health
Readiness |
 |
The Health
District will be participating in three exercises/drills
to test its emergency response protocols. On August 23rd
the Health District had its first internal staff
exercise which helped to review emergency plans and
identify areas for future improvements. On September
24th, in collaboration with the UT School of Public
Health and a company called AudioNet, the Health
District will participate in an exercise involving
response partners including UTMB, Mainland Medical
Center, the Galveston County Office of Emergency
Management, the Galveston County Sheriff's Department,
Gulf Coast Center (MHMR), Red Cross, etc. Participants
will take part in an innovative the two hour exercise
via the internet and audio-conference. The Health
District will be the first public health entity in the
state to use this technology to test its readiness. In
addition, the Health District will participate in a
countywide readiness exercise lead by the Texas
Engineering and Extension Service in summer or fall of

Health Plan Presented at National Conference

On July 31, 2004, Board
Member, Mary Anne Holcomb; Chief Executive Officer, Dr.
Mark Guidry; and Health District Planner, Brian
Rutherford presented information about effectively
conducting a strategic planning process to attendees of
the National Association of Local Boards of Health
(NALBOH) in Denver, Colorado. They discussed planning
philosophies, the importance of having a health plan,
how to facilitate the planning process, and shared the
eight priorities developed through the health district's

Animal Services Adoptathon |

The first ever Galveston
County Animal Shelter Adoptathon brought over 100 people
to the shelter from noon to 7:00 pm on Saturday, August
21, 2004 to view the animals for adoption. The event was
sponsored by the Friends of the Galveston County Animal
Shelter, a non-profit volunteer organization dedicated
to assisting in assuring a safe place for animals at the
shelter. Twenty-three (23) animals were adopted during
the event. Several booths were set up and provided
information from spays and neuter to the ethical
treatment of all animals. Door prizes and a silent
auction were incentives to attendees. The Health
District appreciates the support of the Friends of the
Galveston County Animal Shelter and of board member,
Mary Anne Holcomb who attended the event, and of the
Animal services employees who worked hard to make the

On July 26, 2004 the
Galveston County Mosquito Control office notified the
District that West Nile Virus had been detected in a
pool of mosquitoes on Galveston Island. This was the
first positive identification of WNV in the county. A
Public Health Advisory was issued urging all residents
to follow the 4 D’s to reduce the risk of acquiring WNV.

On July 29, 2004 Galveston

contacted the Public Information Office on July 13, 2004
for information on a story they were preparing advising
citizens how to deal with the extreme heat that was
forecast for that week. Kurt Koopmann, Public

July 30,
2004 the District received notice from the Bolivar
Peninsula Special Utilities District that their water
system had been compromised by a main line break.
Residents were to boil water for safety until further
notice. The Galveston County Health District issued a
Public Health Advisory urging people to boil drinking
water and provided tips on how to successfully do such.

Community Health Center Week the District focused
activities on Diabetes Self-Management. Community
activities were conducted at both the Texas City and
Galveston 4 C’s Clinics with information valuable to

A press release was issued

Governing Board
Chair Resigns - Thelma Logan, a 4C’s Governing
board member and former Health District employee,
resigned from the board on August 25, 2004. She has been
a member since 1996 and served as Chair from 1999-2004.
She will be remembered as an active and supportive
member and board chair and she will be missed. Ms. Logan

The Clinic redesign
project is moving forward. On 8/16/04 Dr.
Guidry and Jay Holland participated in a conference call
with Roger Coleman of Coleman Associates Redesign
Training Programs to discuss the 4C’s redesign
proposals. Coleman Associates had proposed two
approaches to the redesign process: one proposal was to
train, over a six months period, a redesign team(s) who
then would test and implement redesign changes; a second
approach was to take a site and over a 6 weeks period
work with all staff at that site to develop and
implement change. At the next meeting of the Clinic
Leadership Team, after the conference call, Dr. Guidry
and Jay reported to the CLT that they would recommend
the six (6) month approach to redesign. The reasons for
this recommendation included such items as: teams from
both clinics could be trained at the same time thus
insuring that redesign processes in both clinics would
be the same; this method of redesign would insure
sustainability of changes; and that this approach was
the least expensive. This recommendation will be
forwarded to the Governing Board at their September 2004

Two surveyors from the
Health Resources and Services Administration (HRSA), the
agency that oversees our federal 4 C's grant, will be
conducting a HRSA Performance Review of
the clinic from September 21-23. This review, focusing
on organizational performance, is a new process for the
agency and we are one of the first community health
centers to be surveyed. At a minimum the reviewers will
be looking at our senior health initiative and our
dental clinical services. We are currently gathering
data to send to the surveyors on these programs. For

Fiscal Year 2005
Competitive Grant Renewal - The 4C's grant for
Fiscal year '05 is a competitive renewal grant, meaning
that other interested parties in the area may submit a
grant to compete with the 4C's for funding. To assure
that our grant application is the best possible, and
with the Governing Board’s approval of $12,000, the 4C's
has hired The Grant Writing Team, Inc., to assist in the
production of the grant. Jo Carcedo, President of the
company has extensive experience in grant writing and
has recently submitted several successful grant
proposals for new access point Community Health Centers
in the Houston area. The goal is to have the body of the
document and budget ready for the September Governing
Board meeting for review and approval. For more

Nurseline pilot
project
- the Nurseline pilot is completing its second month of
operation. Calls continue to increase to the Nurseline.
Patient comments indicate that the Nurseline is a
valuable addition to 4C's services. It is expected that
the Nurseline pilot will be continued for another month,
pending approval of the Governing Board at the August

Automated Referral
System - The 4C's has instituted an automated
referral tracking system effective August 1, 2004. The
system is designed to allow 4C's staff to track
referrals from their initial entry, through return of
medical records and results. Referral staff received
training in use of the system during the last week of
July. The system is still being monitored for accuracy
and proper use so that data will be accurate,
meaningful, and regularly monitored to assure that 4C’s
patients receive timely referrals and subsequent medical

Air Investigations
Complete - The Air Pollution Services Section
has completed all of the objectives included in their
air compliance contract with the Texas Commission on
Environmental Quality (TCEQ). Air Pollution Services
contracts with TCEQ to fund the majority of the air
pollution investigative activities in Galveston County.
Under this agreement, investigations are conducted to
determine compliance with state and federal air rules
and regulations and air permit provisions. In the FY
2004 contract investigations were conducted at
petrochemical facilities, shipbuilding and ship repair
facilities, gasoline stations and bulk tank terminals.
Facilities found to be not in compliance with the
regulations were given notice to comply and in some
cases, referred to the TCEQ for formal enforcement. For

High Island Sewer
Issues - Work continues on the Health
District’s effort to resolve the sewage disposal issues
in High Island. Representatives of the Texas Commission
on Environmental Quality (TCEQ) along with Health
District staff collected wastewater samples from the
sewage treatment device currently located in High
Island. Laboratory analysis data suggests that the
discharge coming from the outfall of the device is
similar to raw sewage that is received at public
wastewater treatment plants. An attorney with the
Houston-Galveston Area Council has taken the lead in
researching funding alternatives to assist this
community with paying for a collection system and
treatment plant designed to meet today’s standard. One
of the next steps includes assembling community leaders,
government officials, and the peninsula utility district
to decide on the most feasible solution to this problem.
In the meantime, Health District officials will continue
work on identifying all of the residences in the
community, investigate current sewage disposal methods
(i.e. septic system), locate sewage lines that connect
homes to the current sewage disposal device and continue
to communicate with the citizens as we proceed. For more

Beach Watch Signs
- In mid August, the long-awaited Texas Beach Watch
signs were delivered to the Health District from the
Texas General Land Office (GLO). This beach water
monitoring and notification program under the GLO
requires the posting of signs during periods of elevated
bacterial contamination. There are currently 46 sample
stations along Galveston Island, Bolivar Peninsula and
the Texas Dike. The Beach Watch program was developed in
response to the Beaches Environmental Assessment and
Coastal Health Act of 2000. Signs are currently being

Environmental
Crimes Unit -The Environmental Crimes Unit, a
collaborative effort of law enforcement and public
health, tested their skills at identifying, securing and
removing an illegally disposed boat. The boat was
disposed near the shoreline of Moody Gardens in Offats
Bayou. Once it was determined by Garret Foskit that the
vessel was not discharging any fuel or fuel products,
the hull was lifted with newly acquired swim bladders
purchased with grant funds and removed. The boat was
transported to and secured at a local marina until the
owner was notified, served with citations for improper
disposal and required to properly dispose. As one of its
objectives, the environmental crimes unit will continue
to address the growing problem of derelict boats in
Galveston County waterways. For more information contact

Kid's Health Fest:
Our second annual back-to-school Immunization Clinic,
Kid's Health Fest, was held between July 26 and August
17 at the La Marque site. 3,089 immunizations and 122
tuberculosis skin tests were given during this period,
compared to 3,212 immunizations in 2003. A report of
total immunizations given in August will be available at
the next board meeting. Immunization Program staff and
Community Health Nurses were assisted by volunteers who
did everything from dressing as a clown or tooth fairy
and face painting to translating and helping parents
complete required paperwork. The First Presbyterian
Church donated children’s books. Other community
partners included AmeriGroup, Mainland Medical Center,

Community Supports for Diabetes Self- Management
Project:
In celebration of National Community Health Centers
Week, “Diabetes Day” was held on August 11, 2004.
Clinical activities such as foot, dental, and vision
screening, testing for Hemoglobin A1c, and education on
a variety of topics were provided at both clinic sites.
A total of eighty- four (84) people attended this event.
In the afternoon, educational presentations were held at
the Island Community Center and Mainland Medical Center;
twenty- four (24) people participated in these events.
Community partners that assisted with Diabetes Day
included Mainland Medical Center, UTMB, Jesse Tree,
Coastal Area Health Education Center, Housing Authority
of Galveston, Galveston County Agricultural Extension
Service, Robert Wood Johnson Foundation, and various
Diabetes Class and Support Group members. For more

Response to Shark
Bite - July 27th at 4:40 p.m. Galveston EMS's
Medic 5 responded to a reported shark attack on Seawall
Blvd. near 53rd Street on the beach. Paramedic Richard
Chernosky and EMT Jerry Clark found a 19-year-old female
with a large animal bite wound on her right foot. She
was in severe pain, bleeding, and had severe damage to
that foot. After a four-minute scene time, the patient
was transported to the UTMB Emergency Department with
appropriate advanced life support measures. For more

Greg Kunkel, Chief of
Operations of GAAA, attended a Galveston City
council workshop
on August 17th. The City agreed to continue its
$215,000 subsidy for FY2005. They will also continue to
support the Water Bill Donation Fund as it currently
stands. They would like to gradually decrease the dollar
amount it requests from the water bill customers
starting in FY2006 and increase the subsidy to augment
the loss of revenue that causes. They requested more
information from the City Attorney on forming an
Emergency Services District, realizing that additional
ambulances will be needed for the future for the Island.
Council member Cole requested that GAAA re-look at an
EMS subscription service to add additional revenue to
GAAA. A subscription service is a plan that would allow
household to purchase an annual subscription for EMS.
The subscription discounts the co pay should anyone in
the household use EMS that year. One problem is that
Medicare (our largest payer group) has some limiting
regulations on this program that may make it costly.

Volunteer Program
- Nine new volunteers joined the Health District. Four
(4) were assigned to the Animal shelter, one (1) to
Community Health Nursing and special events, and four to
the 4c's clinic (County Indigent Health Care - 1; Lab -
l; Patients Assistance Program -1; Medical -1). On July
24, 2004, at the Nessler Center in Texas City, Shirley
Carr made a presentation about the volunteer program to

On August 12, 2004, the
Galveston County Medical Response Collaborative
met and established vision and mission statements for
the group. The vision is, "To prevent or minimize
adverse consequences of community health emergencies".
The mission statement adopted by the group is, "To
ensure an effective and comprehensive response to
community health emergencies through collaboration among
member institutions". The group also decided to change
its name to the Galveston County Emergency Response
Collaborative (GCERC) to better reflect the
comprehensive nature of the group that is represented by
the Health District, Emergency Management, UTMB,
Mainland Medical, the Sheriff's Department, Red Cross,

The Health District has
submitted an application to be part of a project called
"Public Health Ready". To successfully complete
the program and receive recognition as being public
health ready, the health district will need to meet
criteria in three areas: emergency preparedness
planning, workforce competency training, and
demonstration of readiness through exercises and

On August 6,
2004, the Health District signed a contract for $344,972
with Texas Department of State Health Service
(TDSHS), Office of Public Health Practice for
State General Revenue and Federal pass through monies to
fund local public health activities. There is no change
in total dollars from the previous year. The one change
is that state assigned positions are now included in the
budget as direct assistance. This is a result of
consolidation of HHS agencies and associated state
mandated “retirements” and RIF’s. For additional

On August 16,
2004, the Health District signed a contract amendment
with TDSHS STD/HIV to transfer four (4)
months of the STD/HIV Program Manager’s salary from
direct assistance to financial assistance. The amount
transferred will fund the salary through the end of the
contact term and is a result of state mandated
“retirements” and reduction in forces (RIFs). For

On August 23,
2004, the Health District signed contract amendment #3
for $40,963 with Texas Commission on Environmental
Quality (TCEQ) for the Biowatch Program grant.
This amendment increases the total dollars allocated to
the District without a change in the Scope of Services.
The grant period impacted by this amendment runs through

On August 23,
2004, the Health District signed a contract with
TDSHS for the provision of WIC services to
eligible Galveston County residents. The District will
be paid $9.28 per participant. There was no change to
the contract requirements from the previous year. For
