Your 4C’s pharmacies are staffed by a dedicated group of quality registered pharmacists and certified pharmacy technicians. They are here to get the right medications for qualified and registered 4C's patients. They will answer any questions about your medications prescribed at the 4C's Clinics.

There are a few things you need to remember when using the 4C’s pharmacy. 

  • The 4C’s pharmacy can only fill prescriptions from 4C’s providers (either medical or dental) Although the ‘formulary’ of medications that we have is limited, we will do everything we can to find the medication that will be the least costly to you. The cost is determined on a sliding scale according to your financial information.  In addition, there is a small service charge for each prescription (new and/or refill).
     

  • If there are medications that your doctor wishes you to have but our pharmacy does not carry, then we can refer you to our Patient Assistance Program (PAP) (409-949-3405). The PAP is here to help you get those medications that may be unavailable otherwise. This is done through the drug companies themselves and, if you qualify, these medications can be at no cost to you!
     

  • Also, remember, you can call in a refill order to our pharmacies. Don’t run out of medications! Call in your refill one week before you run out. You can call our pharmacies during clinic hours to see if your medications are ready to be picked up.
