On the 2nd Wednesday of each month the clinics (medical and dental) open at 1:00pm

Welcome to the 4C's clinic website! Here you will find information that will inform you of the array of clinical services 4C’s has to offer you. The 4C's clinics are part of the Galveston County Health District and classified as Federally Qualified Health Centers (FQHC's). Over 50% of the members of the Governing Board is made up of patients!

What the 4C's can do for you:
We have two clinic locations--one in Texas City and one on Galveston Island. We offer quality health services, provided by a qualified, credentialed staff of doctors, dentists, nurse practitioners, physician assistants and dental hygienists. Services we provide include:

Basic primary care for all ages
Low Risk prenatal clinic
Pediatric - well and sick child visits
Women's health exams - pap smear & family planning
Basic dental care
Denture service (by contract)
Counseling for mild mental health conditions

Support services:

On site contract pharmacy for registered 4C's patients
Referrals for specialized care- Hospitalization, deliveries, mammograms, diabetes education
Case Management Service to assist patient with social needs
