Q:  Can anyone see providers in the 4C’s clinics?
A: Any Galveston County resident, with or without insurance, may seek care in the clinics.  Services are, however, limited to Galveston County residents.
Q: Are the 4C’s clinics free?
A: No.  The 4C’s clinics provide discounted healthcare subsidized by federal and county funds.  The amount you will be charged varies based on your personal financial status as well as the need for medications and diagnostics tests.  
Q:  How is my financial status determined?
A: Financial status is determined by income verification which determines your percent of poverty according to federal guidelines. There are documents you will need to determine your eligibility.  For information on those documents and what is needed visit our 4C’s Clinics registration information page on our website. 
Q: If I can not pay, will the 4C’s clinic refuse to serve me?
A: No.  As a federally-qualified community health center, patients are not refused services due to an inability to pay. However, you will be billed for services rendered.
Q: I hear about the Galveston County Indigent Care program, can I get on that if I can’t pay?
A: Galveston County residents who are determined by 4C's to be at 21% of poverty are enrolled in the County Indigent Healthcare program, which does not pay for their primary care in the 4C’s clinics. However, it does pay for their care should they require hospitalization or referral to a specialist.  Primary care in the 4C’s clinics is covered by federal and county funds allocated to the Health District and charges to the patient.
Q: How do I know if I am at 21% of the poverty level?
A: 4C’s registration personnel will conduct a financial screening to determine your status based on a process called “asset testing”.  To be declared at 21% of the poverty level a family of four must have an annual income of $4,452 or less. 
Q:  I have just been discharged from the hospital and need follow up care, is there anyone that can help me?
A:  Yes, The 4C’s clinics have a Case Manager that is jointly funded by the Galveston County Health District, the University of Texas Medical Branch, and Mainland Center Hospital to assist patients seeking care in the clinics. A case manager can be reached at (409) 938-2340.
Q: Will I receive good care in the 4C’s Clinics?
A: Yes.  The Galveston County Health District’s 4C’s clinics provide quality healthcare.  The clinics have maintained JCAHO (Joint Commission on Accreditation of Healthcare Organizations) accreditation since December 1999.  According to JCAHO surveyors, most Federally Qualified Community Health Centers are not JCAHO accredited.  Having this accreditation is an assurance that the 4C’s Clinics meet quality standards and national safety goals.
Q: If I have insurance can I use the 4C’s Clinics for my healthcare?
A: Yes.  The clinics are here for all residents of Galveston County – uninsured and insured.  If you have insurance and the 4C’s clinic has a contract with your health plan, then you will be seen following the agreement with your health plan.  If you have insurance and the 4C’s clinic does not have an agreement with your health plan, contact your health plan for guidance.  You may still be seen at the 4C’s clinic at a discounted rate: however, you will be responsible for payment to the 4C’s clinic and then you may seek reimbursement from your health plan.
Q: Can I walk in and see a provider?
A: Yes.  We will try our best to see you.  We recognize that many patients seek healthcare when they are sick and need quick access to non-emergency primary care.  The 4C’s clinics have some capacity but it varies on a daily basis, based on how many walk-ins arrive, how many providers are on duty, and the complexity of the patient visits that day.  We do have designated providers on Wednesdays to see nothing but walk-in patients.  If you want to discuss your medical needs to see a provider, contact our NurseLine at (409) 938-2355 or (281) 309-0255 who will provide you with best available options.
