On the 2nd Wednesday of each month the clinics (medical and dental) open at 1:00pm. 
 

How to Register to Become a 4C's Patient

The Galveston County 4C’s Clinics provide discounted medical and dental services to qualified Galveston County residents. Whether you qualify for discounted services depends on your income, household composition, and other insurance coverage you currently receive or could receive.In order to qualify you must complete an Application for Discounted Services.You may still access the clinic without completing this application, butyou will be responsible for 100% of charges at the time of service

You may turn in or mail back your application, even if you cannot answer all the questions.  However, if you do not answer all questions your application will be incomplete and the 4C’s staff will be unable to determine if you are eligible for discounted services.

Try to answer as many questions as you can on this application.  Be sure to:

  • Answer all questions the best you can and;

  • Sign and date the last page of the application.  

  • Attach all required documents to application. 

Be sure to review the Application for Discounted Services Checklist before you turn in or mail your application.

Once your application is received, reviewed and approved, you will receive notification of your discounted services rate. 
At your clinic visit you will be required to sign several documents to complete the registration process. 
 

                                                 Apply For Title V Coverage For Your Child's 4C's Visits

Title V Coverage Expands Medical & Dental Services in 4C’s Clinics
After being awarded Title V grant funding, the 4C’s clinic will start registering children 0-21 for Title V coverage of medical and dental services on January 2nd. Title V will cover children who are not eligible for Medicaid in households below 185%
